Following an interview with Dr. Elsbree, your specific medical and wellness needs will be assessed. Should you be accepted as a Hybrid patient, a membership fee will be collected annually. 

This annual fee will ensure you retain Dr. Elsbree's complete medical supervision and comprehensive primary care.
